Back Rolls and Loose Skin After Weight Loss

Loose skin on the back after weight loss forms horizontal rolls across the upper and mid back, often made obvious by a bra band. It is one of the least discussed results of major weight loss and one of the most visible in fitted clothing.

It is skin, not fat, which is why it does not respond to training and why liposuction alone rarely helps.

Why the back behaves this way

Back skin is thick and, once stretched, retracts poorly. After weight loss the underlying volume goes and the envelope remains, settling into folds. A bra band compresses one of them, which is why patients often describe it specifically as a bra roll.

What corrects it

Approach When it applies
Direct excision A discrete roll, with the scar placed where a bra band covers it
Upper body lift Looseness continuing around the trunk and onto the chest wall
Extension of breast surgery Where the roll runs continuously from the breast towards the back
Liposuction Only where it is genuinely fat with good skin — uncommon after major weight loss

The scar

Horizontal, across the back, positioned to sit under a bra band or waistband. Back skin is thick and under tension with every movement, so scars here widen more readily than abdominal ones. That is the honest trade, and Dr Kumar will mark the line on you before you consent. How the scars settle.

It is usually not treated alone

Back rolls sit within a continuous band of looseness that runs around the upper trunk. Treating one roll and leaving the rest around the sides can look worse than leaving it alone. Ask for a plan for the whole trunk, and ask specifically what is planned for your back — it is the area most often left out of a plan focused on the abdomen and breasts.

It combines well with breast surgery or an upper body lift, because the incisions meet. What can be combined.

Medicare

Item 30169 covers removal of redundant non-abdominal skin for functional problems following significant weight loss. A circumferential procedure may fall under item 30179. Functional problems means recurrent rashes or skin breakdown in a fold, or interference with daily activities. The full criteria.

Recovery

Two weeks off work, a gradual return over the following four weeks, full clearance at around twelve. Drains are used and a compression garment is worn for six weeks. Sleeping is the awkward part for the first fortnight — propped or on your side rather than flat on your back. The week-by-week timeline.

Common questions

Will back exercises help? They build muscle underneath. They do not shrink stretched skin.

Can liposuction do it? Only where it is genuinely fat with good skin quality.

Will the scar show? Under a bra band, largely not. Back scars do widen more readily than abdominal ones.

Can it be done with a breast lift? Frequently, and that is often the most efficient plan.

When am I ready? Weight stable six months, nutrition addressed. Background at My Weight Loss Journey.
