After major weight loss, the complaint is usually not the labia minora on their own. It is the whole area: a mons pubis that hangs and overhangs, loose tissue at the outer labia, and skin that has lost its support. Treating the labia minora alone in that situation rarely resolves the symptoms, because the problem sits above and around them.
This is the distinction worth understanding before you book anything.
What changes after significant weight loss
| Area | What happens | What addresses it |
|---|---|---|
| Mons pubis | Descends and overhangs; can obscure the area and cause hygiene and clothing problems | Monsplasty, usually as part of an abdominoplasty or body lift |
| Labia majora (outer) | Deflate and become loose or wrinkled as fat is lost | Excision of the excess, sometimes with fat grafting |
| Labia minora (inner) | May appear more prominent once surrounding tissue has deflated | Labiaplasty |
| Skin quality generally | Reduced elasticity; poor retraction | Excision rather than any tightening treatment |
Why the mons usually comes first
A heavy, overhanging mons produces most of the functional symptoms patients describe — moisture and rashes in the fold, difficulty with hygiene, discomfort with clothing and exercise, and self-consciousness. It also sits directly above the labia, so correcting it changes the appearance and comfort of the whole area.
Dr Kumar performs monsplasty as part of an abdominoplasty or body lift at Westmead Private Hospital, which is the usual sequence: the mons is addressed at the same time as the abdomen, and any labial surgery is considered afterwards, once things have settled. Mons pubis changes after weight loss.
Timing
- Weight stable for at least three months. The relevant Medicare items for abdominal and non-abdominal skin excision require it, and continuing weight loss changes the result.
- Nutrition addressed. Protein, iron, B12 and vitamin D are commonly low after bariatric surgery and wound healing depends on them. Dr Kumar’s practice offers dietitian input before surgery.
- Nicotine. Complete abstinence from all nicotine, not just smoking, for eight weeks before and after body contouring after weight loss.
- The whole plan first. Most patients need more than one operation, and the order matters. Staged body contouring.
Medicare
Labiaplasty has its own item, 35534 — vulvoplasty or labioplasty in a patient aged 18 or more, performed by a specialist, for a structural abnormality causing significant functional impairment.
The mons and abdominal components fall under the body contouring items where their criteria are met — item 30177 (lipectomy with radical abdominoplasty after significant weight loss; intertrigo or another skin condition failing three months of conventional treatment, interference with daily living, weight stable six months;) or item 30166 (wedge excision after weight loss of at least five BMI points, stable six months;). MBS figures as at 1 July 2026, surgeon’s fee only. Your quote itemises each component. More about costs.
Recovery
Labial surgery alone is a day case with swelling settling over two to four weeks and the appearance close to final at about three months. Where it forms part of an abdominoplasty or body lift, plan on the body contouring timeline: two weeks off work, a gradual return over the following four, full clearance at around twelve weeks. Drains are used in body contouring surgery.
Common questions
Can it all be done at once? Sometimes. It depends on the extent of the abdominal and mons work and on whether combining is safe in your case.
Will losing more weight help? If you are still losing, wait. Both the mons and the labia will change further.
Is fat grafting used? Occasionally, for deflated outer labia, where adding volume gives a better result than excision alone.
Do I have to have the mons done? No, but if it is the source of your symptoms, labial surgery alone is unlikely to resolve them.
Can I be assessed remotely first? You can send photos before travelling, which many patients from Orange and further do.
